The "WHO-DAT" is the "Worthwhile Health Outcomes - Dietary Assesment Tool" created by KC Stober (TUSOM 2011) during his Family Medicine rotation in Pierre Part. The Assesment is not by any means a nutritional guide on its own. The results from this brief questionnaire should be used as a tool for discussion between you and your primary care physician to help you develop good eating habits!
